After you have booked an appointment, please take a moment to complete our patient registration form, which is also available as a PDF download for convenient electronic completion. If possible, kindly submit the completed form before your appointment. Should you prefer to fill it out in person, please arrive 10-15 minutes early to complete the necessary paperwork.

What to Bring with You:

• Referral from a GP or specialist
• Medicare, Private Health Fund, or DVA cards
• The location of any relevant X-rays, MRIs, CTs, or other scans that have been performed (e.g Lumus, I-Med Radiology)
• A list of current medications
• Insurance claim information

Consultation Payment Information Full payment for your consultation is required on the day of your visit. We accept payment via EFTPOS, Visa, MasterCard, or cash. If you have a valid referral, your claim will be submitted to Medicare on the day for quick processing (patients should note that private health funds do not contribute to specialist consultations). We can submit your claim to Medicare electronically for quick rebate processing.

Appointment Changes If you cannot attend, please inform us as early as possible so we can reschedule and offer your slot to another patient.
